Your First Visit
When you arrive for your first visit to the clinic, you will need to register with the hospital. It is therefore necessary to bring with you details of your insurance policy and any authorisation numbers that you may have been issued by them.
To help me manage your condition most efficiently, if would be helpful if you bring a number of documents with you. These would include:
- A referral letter from your GP or other health professional.
- Any copies of previous investigations that you might already have had. These are usually stored on a CD Rom.
- A list of any medications that you might be on.
If you have been referred to me as a second opinion, or if you have already had treatment elsewhere, it would be extremely helpful if you bring any information relating to that previous treatment with you.
As part a directive from the Competition and Markets Authority (CMA), I am required to inform you as to a number of details relating to my private practice. Although a letter will be sent to you under separate cover once you book an appointment, this link to my CMA letter covers the relevant points raised.
